Hypertension in Primary Care – A team approach
(British and Irish Hypertension Society Approved)
Skills and knowledge to enable Healthcare Assistants, Nurses, Community pharmacists and GPs to take a pro-active team-approach towards the diagnosis and management of hypertension.
Hypertension is the most common long-term condition which affects 1:4 adults and despite 7.6 million people having a diagnosis a further approximately 6.8 million (47%) remain undiagnosed.
• Hypertension is a major risk factor for stroke, myocardial infarction, heart failure, chronic kidney disease, cognitive decline and premature death.
• Primary care is central to the management of hypertension, with multiple opportunities to test and adjust treatment recommendations (lifestyle and/or drug therapy)
• Hypertension is frequently unmanaged and levels of non-adherence to treatment are estimated to be between 50-80%.
This day has been designed and is delivered by Judith Magowan, a nurse educator who set up the nurse-lead hypertension clinic at the Okehampton Medical Centre where she works as long-term conditions Lead. Judith is an allied health care professional on the British and Irish Hypertension working party and was a committee member for the recently updated NICE hypertension guidelines.
The whole day is suitable for primary care nurses, GPs and pharmacists. The morning is also relevant to health care assistants with the focus on accurate measurement of blood pressure, appropriate investigations and cardiovascular risk assessment. The afternoon’s programme covers initiation of medication and subsequent titration and add-ons, patient education monitoring and on-going management.
