Diabetes Educator (Part-time – Flexible Worker) Type 2 Diabetes Structured Group Education Programme

Diabetes care is one of the major challenges facing the NHS.  Structured education programmes can help adults with Type 2 diabetes to improve their knowledge and skills and also help to motivate them to take control of their condition and self-manage it effectively.

Sentinel Healthcare CiC have been commissioned to deliver patient education to newly diagnosed Type 2 Diabetes patients since 2014.    Our small education team consists of Nurses and Dietitians who are trained and competent to deliver the principles and content of the programme. 

Sentinel educators deliver the structured programme through a series of webinars or alternatively offer a single 3 hour group session to patients referred into the programme from GP surgeries.

The pathway has recently been redesigned and expanded to include collaboration with Livewell’s ‘One You’ programme, which also offers patients a free 5 week lifestyle course and follow up.

Outcomes are measured on patient satisfaction with the ability to self-manage their Type 2 diabetes after attending the structured education programme.  The programme and educators are quality assured and reviewed by the Derriford Hospital Diabetes Team.

Vacancy :

We currently have vacancies for new Diabetes Educators to join our team.

Applicants should have relevant work experience of working with diabetic patients in a community setting and have excellent presentation skills and be able to demonstrate an understanding of educational theory appropriate to the age and needs of the patients referred into the programme.

Successful applicants will undergo a period of observational learning, followed by supported on-the-job training and a final assessment by the Derriford hospital Diabetes staff.

The educator role would be ideal for interested Practice Nurses looking to supplement their existing post or looking to gain experience of working in a community based setting.  Post hours can be flexible dependent on the applicants’ availability and applicants must be willing to travel in and around the Plymouth and South Devon locality and be digitally competent.
